Clinical Excellence Awards

The Clinical Affairs Committee (CAC) would like to honor individuals with Clinical Excellence Awards who have demonstrated excellence in clinical care across all our schools and practice care sites. This will be done by recognizing two individuals, one at the junior (less than 5 years) and one at the senior (more than 5 years) level, from each of our schools (SOM, SOD, SON, SOP). Nominees must be practicing at the minimal clinical effort based on school and meet key attributes below.
 
Letter of support for the nominee can be authored by one or more faculty.
 
Recipients of the award will receive an honorarium of $1000.

Eligibility

  • 8 awards (two per school) - one for junior (<= 5 yrs) and one for senior clinical faculty (>5yrs)
  • $1000 per recipient  
  • Open to faculty of all series, levels of seniority, schools, and practice sites that provide clinical care 
  • Clinical effort eligibility will depend on what clinical effort is representative of the majority of clinical faculty from School of Medicine (SOM), School of Dentistry (SOD) and School of Nursing (SON)
  • Given that the School of Pharmacy (SOP) has a large clinical faculty appointments without salary (WOS) cohort providing clinical care, award recipient(s) can include WOS faculty who meet defined nominee attributes.

Call for Nominations

The Nomination Process:
Eligibility: This award is open to all UCSF appointed faculty (across all series, levels of seniority, schools, and practice sites) who are actively practicing clinical care at a minimum of:
SOM: 40%
SON: 20%
SOD: 40%
SOP: 20%

  1. Nominee’s clinical achievements: please use the “Nomination Form” (found below) to describe the nominee’s clinical activities and achievements. 
  2. Nominator: must be a faculty member who can make a strong assessment of the nominee’s clinical skills. Nominations can be made by multiple faculty; if this is the case one nomination letter should be written and signed by all nominators. Nomination will require endorsement by the Departmental Chief\Chair.

Key Attributes of Considered Nominees:

  • Demonstrates excellent clinical acumen including diagnostic acumen, clinical expertise, and ability to expertly counsel patients/families.
  • Someone who seeks out new knowledge to expand their own knowledge base in service of clinical care.
  • Demonstrates excellent interpersonal skills when working with patients as well as all team members and staff.
  • Commitment to innovation within the clinical realm, whether through implementing improved clinical pathways, streamlining patient care experiences, undertaking novel quality improvement initiatives, or the like.

Nomination Form

Nominators must complete the Clinical Excellence Awards Nomination Form and submit the form in the nomination packet in Senate Service Portal.

Nomination Letter Requirements

  • Submit 1 letter in addition to the short nomination form above. 
  • The letter can be authored by one or multiple fellow faculty members. 
  • The letter/nomination must be endorsed by the nominee’s Chief\Chair.
  • 2-page limit – font size 11, borders 1”, signatures of nominators/chair do not count toward the page limit.
  • Letter must include the outstanding qualities as a clinician. Examples can include: 
    • awards received;
    • creation of patient care programs or activities;
    • creation of innovative quality care programs for improving patient care outcomes; 
    • patient satisfaction (e.g., patient comments or testimonials); 
    • ability to work well in a team environment; 
    • recommendations from team members at different levels/specialties/roles, seen as a clinical role model by peers;
    • contributions to diversity, equity, and inclusion (DEI);
    • specific examples/anecdotes/stories are encouraged.

  • 1. Download and prepare the Nomination Form.
  • 2. Prepare a nomination packet. Nomination packets must include nomination letter and nomination form as one single PDF document.
  • 3. Login to the Academic Senate Service Portal, select Awards/Nominate and submit the nomination packet.
  • 4. Deadline is Tuesday, October 1, 2024.
